By downloading, installing, or using the FormLens iOS application ("App"), you agree to be bound by these Terms of Use.

Description of service

FormLens is an iOS platform that pairs video recording with artificial-intelligence-driven analysis of physical movement and technique. The service uses automated systems to evaluate user-provided video content.

User accounts

Access to certain App functions requires authentication via Sign in with Apple. You are responsible for keeping your login credentials secure and for any actions taken through your account.

User content

You retain ownership of any videos and content you create using the App. You grant FormLens a limited license to process your content solely for the purpose of providing the App's features.

You agree not to submit content that is illegal, harmful, or violates the rights of others.

Subscriptions and payments

• The Apple App Store handles subscription processing
• Charges are applied to your Apple ID at confirmation of purchase
• Subscriptions automatically renew unless cancelled at least 24 hours before the end of the current period
• Management and cancellation occur through your App Store account settings
• Refunds follow Apple's established policies

Disclaimer of warranties

The App and AI-generated feedback are provided "as is" without warranties of any kind. FormLens makes no guarantees regarding the accuracy or completeness of analysis and is not a substitute for professional coaching, medical advice, or physical training guidance.

Limitation of liability

FormLens disclaims responsibility for indirect, incidental, special, or consequential damages resulting from use of the App.

Termination

FormLens reserves the right to suspend or terminate access for policy violations or other reasons.

Changes to terms

These Terms may be updated periodically. Continued use of the App signals acceptance of the updated Terms.
